It's at the Heart of What We Do. *Life as a Hospice RN Case Manager*_Role of the RN Case Manager_ As a Hospice Case Manager, you will have a special opportunity to provide, coordinate and direct the provision of hospice nursing and palliative care to terminally ill patients in their home and wherever they call home. Our Hospice nurses will closely collaborate with the attending physician, patients and their families, and other members of the patient care team to provide and maintain continuity of patient care to achieve excellent pain and symptom management and high-quality end-of-life care for the patient. We service an adult population providing support to patients and families. _Nuts & Bolts of the Job_ Hospice RN Case Managers work Monday - Friday with a potential on call and Holiday coverage schedule. Case Manage 15-18 patients, attends Interdisciplinary Team Meetings, contributes to team huddle calls, and receives updates on your patients daily. Provide Case Management: * Assign Aide Visits
* Coordination of Care with Social Workers, Chaplains, Volunteers and Bereavement team
