JOB PURPOSE:

With precise planning and expert supervision, they oversee clinical
operations at their designated location, directly accountable to
the Home Health Director. Our Manager ensures that a dedicated team
of associates, including skilled nurses, therapists, social
workers, and home health aides delivers services aligned with the

ESSENTIAL FUNCTIONS:
1. Manages day-to-day operational functions of the assigned
location to ensure standards of patient care and service delivery,
staff productivity, operational and budget targets are met.
2. Protects the assets of the agency and works with Agency
leadership to ensure appropriate utilization of resources to
prevent waste, fraud and abuse.
3. Works with referral sources to enhance service provision to
patients, assure safe transitions of care and enhance population
health.
4. Serves as the subject matter expert on assigned areas including
the electronic medical record functionality, agency policies and
procedures, accreditation standards and laws and regulations in
order to develop and retain highly qualified Associates.
5. Ensures customer-focused service to external as well as internal
customers, including Associates.
6. Actively holds Associates accountable through effective coaching
and counseling techniques that enhance staff engagement. Assists
the Home Health Director with any disciplinary actions when
performance or behavioral expectations are not met.
7. Activity works with the agencys Business Development Liaison to
build relationships within the service area to promote agency
growth and increase positive agency and company reputation.
8. Works with Agency Leadership to innovate, improve operational
performance and enhance patient quality, safety and
satisfaction.
9. Provides routine on-call coverage.
10. May assist in providing patient visits or cover nurse call
unique to the assigned location.
11. Performs other duties as assigned.

Qualifications:
MINIMUM EDUCATION REQUIRED:
Graduate of a Nursing Program.
MINIMUM EXPERIENCE REQUIRED:
Minimum five years of clinical experience, two of which must be
recent in home health, plus two years of progressive management
experience required (for total of seven years), preferably in home
health which could include back office operations of home health
quality management, staff orientation and education, patient
intake, scheduling and staff management or supervision.
MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:
Current License in the State of Georgia as a Registered Nurse or
NLC/eNLC Multistate License.
Valid Georgia Drivers license.
ADDITIONAL QUALIFICATIONS:
Knowledge of Medicare, Medicaid, and accrediting organizations
regulations and standards required.
OASIS experience required.
