• School-based health center**SummaryUnder the general supervision of a staff physician and within the established protocols, the Nurse Practitioner performs assessment and treatment of chronic and acute medical conditions. Tasks delegated to be performed by the Nurse Practitioner will be adequately monitored by the supervising physician, who will take full responsibility for these tasks. As a recognized patient-centered medical home (PCMH), every employee is responsible for ensuring the successful implementation of quality, comprehensive patient-centered care as a member of the patients care team. By performing the job expectations described below you will be an active member of the care team thereby patients are assured access to team-based, coordinated, comprehensive, quality care.ESSENTIAL DUTIES AND RESPONSIBILITIES (include, but are not limited to, the following)
  • Adheres to the established protocols for standards and limits to clinical care delivery
  • Obtains a detailed patient history and records the finding in the patient record
  • Performs systematic and thorough physical examinations
  • Orders, performs or schedules laboratory and specialty diagnostic studies
  • Telephones prescription orders with the exception of Schedule II drugs
  • Prescribes with DEA number written prescription orders schedule III - V
  • Performs emergency care
  • Provides patient counseling on health habits, exercise, tobacco, alcohol, etc.
  • Provides supportive counseling as needed
  • Provides patient education on diet, use of physical adjuncts to therapy (nebulizer, vaporizer, etc.), prenatal instructions, child care instructions, etc.
  • Telephone contact with patients for follow-up and triage
  • Participates in community health promotion and disease prevention projects
  • Represents center at various professional and community functions
  • Participates in the corporations Quality Assurance program
  • Other duties as may be assigned.Supervisory ResponsibilitiesProvides consultation and direction to supportive clinical personnel.QualificationsTo perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
