DescriptionJOB PURPOSE:Responsible for providing indirect patient care by ensuring that the list of home medications is completed per Policy and Procedure at the time of admission and assisting physicians and midlevel providers with transfer/discharge medication reconciliation, when requested.#IND123#GDQualificationsMINIMUM EDUCATION REQUIRED:Graduate of a nursing programMinimum Experience RequiredTwo (2) years experience in an Inpatient Nursing UnitMinimum Licensure/Certification Required By LawCurrent License in the State of Georgia as a Registered Nurse or NLC/eNLC Multistate License.
