Job Title: Registered Nurse Job Specialty: Home Health Job Duration: 13 weeks Shift: Day 5x8-Hour (08:00 - 17:00) Guaranteed Hours: Not guaranteed Experience: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience License: Must have license in hand Certifications: CPR, BLS Must-Have: - Proficiency in HomeCare HomeBase EMR and OASIS - Must have a reliable vehicle, driver's license, and auto insurance - Ability to travel up to 75 miles from the assigned care center Job Description: - Provide, coordinate, and direct the provision of home nursing care according to physicians' orders based on agency policies and procedures. - Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. - Conduct home visits and attend periodic office meetings. - Manage a daily average of 5-8 patients, equating to roughly 6-8 points per day. - Handle various types of cases including Start of Care, Wound Care, Follow-up Visits, etc. - Complete all documentation within 24 hours of the patient appointment. - Participate in on-call rotations, which typically include 1 weekend per month. - Cover a territory mileage radius of up to 75 miles, with mileage reimbursed at the IRS standard rate. - Engage with the team and visit patients in their homes, with the possibility of floating to nearby clinics if patient census is low.
