Job Description:

A company is looking for a Telehealth Nurse Practitioner or Physician Assistant specializing in Dementia Care.
Key Responsibilities

Evaluate and examine patients with memory complaints
Provide patients with a diagnosis and care plan to guide their health goals
Review labs and imaging relevant to patient care and conduct follow-up appointments

Required Qualifications

Licensed Nurse Practitioner (NP) or Physician Assistant (PA)
Commitment of a minimum of 8 hours per week
ABPN certification or eligibility in Neurology
Empathic, mission-driven, and collaborative culture fit
