Purpose:Mercy Care Atlanta is seeking a compassionate and
detail-oriented Psychiatric RN Care Coordinator to enhance patient
access to resources within our psychiatry practice. This role is
essential in improving health outcomes for individuals receiving
psychiatric services by providing comprehensive care, medication
management, and care coordination.What You Will Do
- Conduct nursing assessments for patients entering the Substance
Use Disorder (SUD) Program and coordinate with the supervising
physician.
- Assist the psychiatry team by initiating medication therapy,
including Long-Acting Injectables (LAIs), and facilitating the
Patient Assistance Program (PAP) enrollment.
- Complete prior authorizations by liaising with insurance
providers, pharmacies, and clients while maintaining meticulous
follow-up.
- Support providers by rooming patients, responding to
psychiatric emergencies, and managing patient registries.
- Oversee medication reception and distribution in coordination
with facilities management staff.
- Handle in-basket messages from providers, clients, and the
access center, ensuring timely responses.
- Providing direct client care, performing mandatory client
screenings, medication administration, monitoring responses to the
medications and client education.
- Assist clients with linkage of care to other resources allowing
the clients to complete the healthcare continuum.
- Participate in community outreach, including health fairs and
events three times per year.
- Adhere to departmental policies and Mercy Care's mission of
serving underserved populations.Minimum Qualifications
- Associate's or Bachelor's Degree in Nursing.
- Active RN license and CPR/BLS certification.
- At least two years of related experience, preferably in an
outpatient care setting.
- Psychiatric experience and a passion for serving disadvantaged
populations are highly desirable.
- Ability to effectively assess and evaluate clients and
