Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience, depending on the specialty. License: Must have valid nursing license in hand. Certifications: CPR, BLS Must-Have: Recent specialty experience is required. Proficiency in HomeCare HomeBase and OASIS is required. Willingness to travel up to 75 miles from the home base is mandatory. Description: Provides, coordinates, and directs the provision of home nursing care according to physician orders. Care is based on agency policies and procedures through the competent application of the nursing process. Responsible to the Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses. Home visits and periodic office meetings are required. Must have reliable transportation, a current driver's license, and agency-required liability insurance. Average number of patients per day is 5-8. Contractor will be assigned between 5-10 patients daily. Each type of visit and activity holds a different point value. If there is a situation where the contractor cannot meet their financial needs due to low patient census, please reach out to the Account Manager for possible reassignment to a nearby clinic. Types of cases include Start of Care, Wound Care, and Follow-up Visits. First-timers are not accepted; recent specialty experience is mandatory. Weekend requirements vary by location and profession. On-call requirements vary by location and profession. The consultant will use HomeCare HomeBase and Oasis for documentation. Candidates must be comfortable with potential travel times between patients. Documentation must be completed within 24 hours following appointments. There are no guaranteed hours with this contract; the clinician must meet weekly productivity points. All overtime must be pre-approved. Consultant must accept every patient on their schedule regardless of distance from previous appointments.
