Up to $265K annual salary $50K Signing bonus Up to $25K in Annual

patient-populatio Preventative care focus on Chronic conditions to
reduce hospitalization Well trusted, Physician-lead healthcare org.
with over 120 locations in 15 states Patient-focused, small patient
panel – 450 patients max Monday – Friday shift-schedule; No
Weekend, No Call Dedicated care team for full PCP support, as well
