In this career, you will help individuals create goals tailored to health and well-being, lifelong learning, community living activities, home living skills, etc. We are looking for passionate, skilled, and dedicated staff to become part of our diverse team and provide day-to-day opportunities which support and guide individuals in achieving personal goals for maximum living.Physical Job Requirements:* Must be able to lift 20 pounds* Requires long periods of sitting/standing* Must have valid Georgia driver's license and Motor Vehicle Record in accordance with company policy* Candidates for selection must pass a criminal background check (including fingerprinting)* Must pass a pre-employment drug screen and subject to random drug screensAny combination of training and experience, which would have enabled the applicant to acquire the necessary knowledge, skills and abilities Note: Some positions may require a valid driver's licenseView Point Health is seeking a Team Leader, who is licensed (LPC or LCSW), to lead the Light Projects Early Treatment Program (ETP) and provide transitional services to clients throughout DeKalb/Fulton Counties.Early Treatment Program is a team based, multi-element approach to treating transition age youth and young adults, ages 16-30 years old with FEP (First Episode Psychosis). This position will be based out of our DeKalb Office. The team of professionals will include a Psychiatrist, Registered Nurse, Employment Specialist, Clinical Team Leader/Individual Therapist, Case Manager, Certified Peer Specialist, & Family Peer Support Specialist.CSC (Coordinated Specialty Care) is a collaborative, recovery-oriented approach involving consumers, treatment team members, family members (when appropriate), as active participants. CSC emphasizes shared decision making as a means for addressing the unique needs, preferences, & recovery goals of individuals served. CSC services are also highly coordinated w/ primary medical care, w/ a focus on optimizing consumers overall mental, emotional, & physical health.Shift: Mon - Fri (1st Shift)Duties & Responsibilities (not limited to):* Interview individual referred family members, educational institutes, and/or support system to assess level of functioning, current Strengths, Needs, Abilities, and Preferences and secure information relative to program requirements for receiving services, recommending appropriate level of care, and authorize services via care logic.* Conduct and hold weekly treatment team meetings to assist in the development of treatment plans with appropriate therapeutic interventions, discuss any unusual observations or incidents, discuss individual strengths, needs, abilities, and preferences; individual progress, and follow up.* Manage and maintain all authorizations, team's census of 30 clients, and staff performance.* Provide individual therapy 2-4 times a month to each individual enrolled depending on current risk factors and history.* Report concerns and needs of the program or individuals served to Program Manager. Incorporate & Execute Programmatic feedback from Program Manager and agency policies.* Recognize the need for crisis intervention and conduct crisis assessments. Coordinate voluntary or involuntary crisis admissions as needed.* Write precise, accurate, and clear progress notes in our electronic Care Logic system in a timely manner. Maintain required records & information in each client's file. Prepares accurate, complete and timely documentation of case notes and billing information to meet agency and state standards and provide effective service delivery across a broad range of services.* Provide individual and group supportive counseling to clients and families concerning their needs and problems, and issues with personal and social development.* Attend interdisciplinary meetings, trainings, and continuing education relation as it relates to the job positionCandidates for selection must pass a criminal background check (including finger-printing), pass a pre-employment drug screen, have a valid Georgia driver's license, and a Motor Vehicle Record in accordance with policy.View Point Health provides health benefits and 401K, in addition to CEU credits to meet licensing requirements.We are an equal opportunity employer.Due to the volume of applications received, we are unable to provide information on application status by phone or e-mail.
