Resident Care Coordinator, you will: be responsible for
coordinating the day-to-day delivery of Activities of Daily Living
(ADLs) and Instrumental Activities of Daily Living (iADLs) by the
community's team of Resident Care Partners (RCPs). You will also
assist the Care Services Manager with scheduling all clinical staff
to ensure resident needs are met while complying with the
community's budget.What you'll do here:Assist the Care Services
Manager with comprehensive resident pre move-in, admission, and
quarterly evaluationsPartner with the Care Services Manager to
implement the individualized care plan for each residentReport and
document community issues through internal and external reporting
processes and conduct internal investigationsParticipate in manager
on duty rotation and provide oversight of the community's
medication management program in the absence of the Care Services
ManagerAssist the Care Services Manager with recruiting,
interviewing, hiring, orienting, and training clinical and other
staffCreate schedules for all clinical staffAssist the Care
Services Manager with community quality improvement processes in
consultation with the Executive DirectorCoordinate with third-party
physicians, pharmacy, and other medical professionals to promote
quality care and independence for residentsEnsure utmost resident
and employee satisfaction and retention through continuous
improvement of care, service, and cultureRegularly assist residents
with medication administration, bathing, toileting, grooming,
dressing, transferring, and eatingPartner with the Care Services
Manager to ensure quality resident care is consistently delivered
in accordance with all local, state, and federal lawPerform other
duties as assignedWe'd love to talk if:You are a licensed nurse
(RN, LPN/LVN) with a current/unencumbered licenseYou are proficient
with Microsoft Office and have the ability to learn new software
applicationsYou meet, have, or are eligible to obtain all
applicable state requirements and certificationsYou have basic
written and oral English communication skillsYou care about serving
seniorsBonus points if:You are a Registered NurseYou have at least
1 year of clinical experience in long term care, community-based
care, or assisted livingYou have experience interacting with
individuals diagnosed with dementia and Alzheimer's diseaseHourly
Pay Range: $22.50 - $25 / Hour, Based on ExperienceWe're also proud
